1	Are you a paid up member of British Fertility Society?	Complete application form and make payment View infromation about BFS Membership

2	Is your training centre recognised by the BFS for providing training? Recognised Trainers/Centres	Enlist proposed centre to apply by completing application form Application for training centre recognition

3	Is your trainer recognised by the BFS for providing training? Recognised Trainers/Centres	Enlist proposed trainer to apply by completing application form Expectations of the trainer (pdf)_(103kb) Application to be a trainer

4	Have you completed the application form for your chosen programme?	Please complete application form and submit your CV Application forms for Trainees

5	Has your proposed trainer completed and provided you with letter of support?	Please enlist your proposed trainer to complete letter of support document Letter of support from trainer

6	Have you included payment for this course? Course fee £120 payable upon application	Please ensure compliance to avoid rejection of your application

7	Are the criteria in 1,2,3,4, 5 and 6 satisfied?	Attendance at the relevant theoretical course is required prior to completion BFS events & courses

8	Please submit completed application forms to the BFS Secretariat Contact BFS
